 Your Health Benefits Program 

 

            The district currently provides the Oxford Freedom Plan to all full time employees and their dependents.  You have the option to use either in-network or out of network providers.  REMINDER:  Please remember to notify human resources when you have a qualifying event that requires a change to your health benefits coverage such as marriage, divorce, or the birth of a child, etc.  It is important that we be notified of these changes within 31 days of the date of the event.  If the individual is not enrolled within this time period they will have to wait until the open enrollment period on July 1st.

To create your own Oxford Member Account go to the link below and select "Need to Register?" - enter the ID on your Oxford identification card along with your name, date of birth, address, etc.  You will be asked to create a login/password.     "Click Here"     In addition to other services, members can check claims status online and request specific informational materials. 

 

 

Opt-Out of Health Benefits 

 

The district has adopted an opt-out of insurance program for all covered employees.  This currently applies to Health Benefits/Prescription only.  If you choose to opt-out of the health benefits, you and your eligible dependents will continue coverage with Delta and Benevision.  You will be reimbursed up to 30% of the premium depending on the opt-out plan you select.  Payments are made through voucher on a quarterly basis.  For more information, please contact Colleen Smith in Human Resources at csmith@hcstonline.org

$6,356 - Opt out of family coverage

$2,716- Opt out of single coverage

$3,644-Opt out of family coverage and elect single coverage
