If you are injured at work it is important that you contact your supervisor immediately if you are filing a claim. An incident report should be filed with QUALCARE at 800-425-3222. You will be required to give specific details as to date, time, and location of injury. In addition, please notify Mary Coluccio, or Colleen Smith that a claim has been filed.
